1. This card is valid for the registered cardholder only and is non-transferable.
2. This card is valid for two (2) years from the date of issuance.
3. This Rahmah Card is exclusively for eligible groups such as B40, senior citizens, single parents, students & persons with disabilities (subject to verification).
4. Cardholders are entitled to selected benefits/discounts for services across all networks under Bloom Healthcare.
5. Benefits provided are subject to current terms and may change without prior notice.
6. The card must be presented prior to receiving any treatment or service.
7. Not valid in conjunction with other promotions unless stated otherwise.
8. Lost or damaged cards must be reported immediately; replacement may be subject to a fee.
9. Misuse of the card may result in termination of membership without notice.
10. Bloom Healthcare reserves the right to amend the terms & conditions at any time.
